Vida Sana and the First 1000 Days: Addressing Toxic Community Stress and Housing Instability in Families with Young Children—A Two Generation Approach

Summary: The goal of this project is to decrease toxic community stress among low income and Hispanic/Latinx families living in mobile home parks in Larimer County by increasing protective local policies for land preservation/designation, facilitating resident ownership of property to increase community stability, and transforming the delivery of community-based trauma informed care and supportive services to families with young children living in these communities.

Creating a Community Health Hub in Northwest Fort Collins

Summary: The Salud Community Health Hub (SCHH) project will create a foundation for the SCHH by piloting community wellness programs focused on chronic disease prevention, building connections to local networks of services to address social determinants of health, and providing healthy design consultation for construction of permanent site amenities. The SCHH will be a demonstration site in which the health care industry incorporates primary prevention and primary care services, while facilitating connection to social services in a single location. These integrated preventative services will provide stable and coordinated support that attends to the health of whole person by developing impactful, culturally-relevant, and proactive health and wellness programs and services.

Milestones: Focus groups in Spanish and English, walking & transit audits, information sessions, and design workshops were delivered as planned for the project. See final presentation here

Future State: A place for community to gather that supports health and wellbeing via integration of preventative health in the system of care and services.
